How this website’s forms work
The appointment and medical-insurance review forms store the information you submit in our website database and send a notification to Asana Dental through our configured email provider. We also record submit-button events and related technical details—including the page, campaign attribution, date and time, IP address, browser information, and referrer—to operate, secure, troubleshoot, and measure the website. The forms are not a patient portal and should not be used for diagnoses, policy or member numbers, Social Security numbers, payment details, or insurance-card images.

When information is protected health information under HIPAA, our uses and disclosures are governed by HIPAA and our Notice of Privacy Practices. This website policy does not replace that notice.
